含钾高渗氯化钠液治疗出血性休克犬对血钾浓度的影响

【摘要】 实验观察合不同浓度氯化钾的高渗氯化钠溶液复苏出血性休克犬对血清K+的影响。结果表明,含低浓度氯化钾的7.5%氯化钠浪静注后,血K+一过性增高,然后逐渐降低,至注射后30分钟时血K+与注射前水平相等。静脉注射合1.5%氯化钾的高渗氯化钠溶液后,观察期内各血K+测定值均显著高于注射前值。7.5%氯化钠溶液能有效地逆转出血性休克。结果提示,较低浓度的氯化钾溶液不足以预防静注高渗氯化钠所致低钾血症,应在严密测血K+基础上,静脉滴注氯化钾溶液以维持血K+在正常水平。

【Abstract】 Hypokalemia following intravenous hypertonic saline has been reported during resucitatlng hemorrhagic Shock (HM). This ex penment was to study the effects of the mixture solutions of 7.5% NaCI and KCI at different concentration in reversing HM. During induction of HM ,8 healthy dogs were bled slowly from the femoral artery until mean arterial pressure decreased to 8 kPa. The intravenous infusion of 7.5% NaCl-0. 5% KCI solution 4ml/kg was given, and after serum potassium level returned to that before the infusion, the intravenous injection of 7.5% NaClsil. 5% KCI solution 4ml/kg was administrated. During the whole procedures the hemodynamic parameters were monitored through Swan-Ganz catheter. The blood samples were collected to measure serum lev els of sodium and potassium, before induction of HM, prior to infusion of hypertonic saline and at lst, sib, 10th and 30th min following the administration of either mixture. The results demonstrated that as compared with the levels before injection of the saline,the K+ level remained statistically unchanged (P>0. 05)except for a transient increase immediately after the injection, following administration of 7.5% NaCl-0.5% KCI solution(P<0. 05),and was ascended to and kept at higher levels after the infusion of 7.5% NaCl-1. 5% KCl solution (P<0. 05). and Na+ levels went up and remained at higher concentrations jollowing both administrations during whole period of observation (P<0. 05). It is suggested that when hypertonic saline is applied to resucitating hemorrhagic shock, supplementing potassium chloride may be necessary to correct the hypokalemia,resulting from Na-K pump’s being activated by the higher level of sodium.
