青心酮对慢性阻塞性肺疾病的治疗机制探讨
Therapeutical mechanism of 3, 4 dihydroxyacetoph-enone in treating chronic obstructive pulmonary dis-ease
【摘要】 在11例接受右心漂浮导管检查的慢性阻塞性肺疾病(慢阻肺)患者中,静脉注射青心酮(DHAP)后肺动脉平均压下降约15%,肺血管阻力下降39%,体循环阻力下降21%,而心输出量上升27%,体动脉血压及血气无改变。放射免疫测定也表明,用药后血浆内皮素1(ETI)及环磷酸鸟苷(cGMP)水平下降(P<0.01,P<0.05)。上述研究表明,青心酮能降低肺血管阻力,其药理机制可能与降低血浆ETI水平及调节环磷酸腺苷/环磷酸鸟苷(cAMP/cGMP)te值有关。
【Abstract】 emodynamic parameters were measured by ballontipped catheter in 11 patients with COPD and plasmalevels of endothelinl (ET1)i , cAMP and cGMP weredetected by radioimmunoassay. The results indicatedthat 3 , 4-dihydroxyacetophenone (DHAP) gives with aniv dose of 640mg could reduce (1) 15% of mean pul-monary arterial pressure, 39% of pulmonary vascularresistance and 21% of systemic vascular resistance ; (2)but not effect on systemic arterial pressure and blood gasanalysis ; (3) decrease the plasma ETI as well as cGMPlevel (P<0. 01, P<0. 05 respectivelyl. The authorsthink that the decrease of pulmonary arterial pressure inCOPD patients may be related to the fowering of plasmaETl ievel and the change of the cAMP to cGMP ratio.
