科雷斯氏骨折后腕骨运动变化的定量分析

【摘要】 采用三维数字化测量系统对科雷斯氏骨折后腕骨运动变化进行了定量分析。结果表明:桡骨远端的背伸成角主要影响腕骨的屈伸运动,而桡骨远端的桡倾丧失通常造成腕骨异常的尺桡偏活动。骨折端的背伸和桡倾畸形主要影响近侧列腕骨的运动。不同程度的骨折畸形对腕骨运动的改变有着绝然不同的作用。科雷斯氏骨折畸形愈合时易发生腕中关节不稳。这种关节不稳是由于月骨追随桡骨远端背伸成角与桡倾丧失的倾斜面,而头状骨则代偿性处在屈曲和尺偏位置所致。

【Abstract】 Carpal kinematics in malunited Colles’fractures were studied in five cadaver wrists with a three-dimensional digitizing system.It was revealed that both dorsal and radial angulation in malunited Colles’fractures had great influence on carpal kinematics. Proximal carpal bone motion was effected most after dorsal and radial angulation.Different degrees of deformity associated with malunited Colles’fractured had different influences on carpal bone motion.Stage Ⅰ and Ⅳ had a little effect,while stage Ⅲ and Ⅵ caused greater alteration on carpal kinematics.Midcarpal motion was significantly increased after dorsal and radial angulation of the distal radius,due to a dramatic flexion and ulnar deviation of the capitate with respect to the lunate.Nondissociative DISI deformity was observed after 34 of dorsal angulation (stage Ⅲ) of the distal radius.
