胃良、恶性病变p53,met基因产物的免疫组化研究

【摘要】 为了研究p53和met基因在胃癌发生中的变化,本文用免疫组化方法检测p53蛋白在胃癌(55例,其中弥漫型27例、肠型28例)与癌旁不典型增生(42例)、肠上皮化生(28例)和正常胃粘膜(49例)的表达,同时检测了met基因产物在胃癌(53例,其中弥漫型27例,肠型26例)与癌旁不典型增生(42例)、肠上皮化生(28例)和正常胃粘膜(47例)的表达。结果表明,p53蛋白在胃癌中表达的阳性率显著高于癌旁不典型增生。肠上皮化生和正常胃粘膜,阳性率与胃癌的组织学类型(弥漫型、肠型)无关。抗met基因产物的单克险抗体S_(19)免疫组化染色的阳性率在胃癌、癌旁不典型增生、肠上皮化生间差别元统计意义(P>0.05)。但这三者均显著高于癌旁正常胃粘膜。抗met基因产物的多克隆抗体C_(28)免疫组化染色阳性率在胃癌、癌旁不典型增生和肠上皮化生间差别亦无统计意义,且前二者显著高于癌旁正常胃粘膜。以上结果提示:p53基础因突变在胃癌(肠型)发生过程中的早期阶段即可出现,是胃癌发生过程中较早期的现象,p53蛋白的表达与胃癌相关病变的发生、发展及恶性转化有关,而met基因产物的过表达则是肠型胃癌发生过程中很早期的现象。

【Abstract】 The expression of p53 protein in gastric carcinoma(55 casee,including 27 diffuse type and 28intestinal type),dysplastic,metaplastic and normal mucasa adjacent to carcinoma,and of met oncoprotein ingastric carcinoma(53 cases,including 27 diffuse type and 26 intestinal type),dysplastic,metaplastic,and nomalmucosa adjacent to carcinoma were detected with immunohistochemistry.The result showed that the positiverate of p53 in carcinoma(54.55%)was higher than dysplasia(25.58%)and intestinai metaplasia(16.67%).The normal mucosa adjacent to the tumor had the lowest positive rate(2.13%).There was no significantdifference between diffuse type and intestinal type.For met oncoprotein,the positivee rate did not showsignificant difference among carcinoma,dysplasia and intestinal metaplasia,howeer,the positie rate of theselesions were significantly higher than normal mucosa. The result suggested that the accumulation of p53protein begin to occur in metaplastic lesions and getting higher and higher with the development of lesions inthe process of gastrocarcinogenesis. So The accumulation of p53 protein is associated with the development ofgastric lesions and it′s malignant transformation. The result of this study and other investigations support theview that activation of C-met is a very early event in gastric carcinogenesis and provide an evidence af therelationship between dysplasia and metaplasia with histogenesis of intestinal type carcinoma There was nospecific expression of met oncoprotein in carcinoma tissues. Detection of this protein may be helpful to findout the lesions that may be associated with carcinoma of the stomach.
