卡介苗防治膀胱肿瘤原位免疫细胞活化的研究

【摘要】 采用免疫组化技术及单克隆抗体Tac(抗IL-2受体)、I2(抗HLA-DR抗原)及OKT9(抗TFR),对17例膀胱肿瘤和23例术后预防肿瘤复发者卡介苗(BCG)灌注后膀胱原位单核细胞中免疫细胞的活化进行研究。BCG灌注后,膀胱粘膜上皮出现大量DR+抗原表达。膀胱粘膜上皮细胞成为抗原递呈细胞,可能是BCG抗膀胱癌重要作用机理之一。BCG灌注后,膀胱粘膜固有层单核细胞中的Tac+、I2+、T9+细胞明显增多,是BCG刺激膀胱局部,使免疫细胞大量激活的标志。膀胱原位免疫细胞活化需要BCG不断地刺激,预防肿瘤复发需长久灌注BCG。

【Abstract】 17 cases of superficial bladder cancer and 23 cases of preventing the recurrence of the cancer were studied by immuno-histochemical technique with a panel of monoclonal antibodies Tac (anti-IL-2R),I2(anti-HLA-DR),and OKT9(anti-TFR),in order to explore the expressions of the activatied immunocytes on bladder local befor and after intravesical BCG. Bladder mucosa epi thelium exhibited large amounts of HLA-DR antigens after intravesical BCG. Urothelial cell becoming antigen-presenting cell (APC) may be one important finding for the mechanisms of BCG resisting bladder cancer. The expressions of the cells of Tac,I2,and T9 on subjacent lamina propria were increased more than that befor intravesical BCG. BCG can stimulate bladder local immunocytes activated. Because the activate of the bladder local immunocytes required BCG stimulating constantly, long term intravesical instillation BCG for proventing the recurrence of bladder cancer is needed.
