【摘要】 我科自１９７８～１９９４年共完成肺叶袖状切除治疗中心型肺癌６３例，按ＴＮＭ分期，Ⅰ期２１例，Ⅱ期９例，Ⅲ＿ａ期３１例，Ⅲ＿ｂ期２例。上叶袖状切除４３例，占６８．２５％，其次是下叶袖状切除。支气管全部用对端吻合法，其中１４例用连续粘膜下吻合效果良好。６３例全部临床治愈，无手术死亡，术后并发症４．７％。术后半年内吻合口狭窄２例，１年后吻合口癌再发２例。术后３和５年生存率分别为６１．１％和４５．０％。在讨论中详细阐述了手术指征和手术方法等。更多还原

【Abstract】 cases of central type carcinoma of lung weve performed sleeve resection oflobes in our departmeat from 1978 to 1994.According to TNM criteria,there were 21 cases instage Ⅰ,9 cases in stageⅡ, 31 cases in stage Ⅲ_a and 2 cases in stage Ⅲ_b. 43 cases were per-formed sleeve resection of upper lobe(68.25%)and the other 20 cases were performed sleeveresection of lower lobe. End-to-end anastomosis was carried on in bronchi,in which 14 casesachieved a satisfactory result by continous submucous anastomosis All 63 cases recoveredwith no operation death.The postoperective complication rate was 4.7%and stoma stenosishappened in 2 cases 6 months later and stoma cancer recurred in 2 cases 1 year later. The sur-vival rates of postoperative 3 and 5 years were 61.1%and 45.0%. The operative indicationand proceedure were proposed in detail in this article.更多还原
