乙肝疫苗与卡介苗同时免疫接种的交互作用

【摘要】 我国是乙型病毒型肝炎的高发区,携带病毒的母亲约有40%可将病毒直接传染给婴儿,所以我国的免疫程序要求婴儿出生后24h之内接种乙肝疫苗(HeVac)。出生后结核感染的早期性,我国一直规定新生儿于出生后尽快接种卡介苗,以上要求均符合WHO扩大免疫计划的要求。两种疫苗在新生儿中同时接种的问题需要认真的研究。我们对周口市几家城市医院出生的新生儿进行了有关研究,探讨了乙型肝炎疫苗与卡介苗(BCG)在新生儿中同时接种时每种疫苗对另一种疫苗在免疫生成方面的交互作用。同时组与单独乙肝疫苗免疫组相比,抗-HBs产生水平于免后1个月分别为27.21049mIU/ml(n=103)及22.01286mIU/ml(n=105),差异不显著(t=1.117734,P>0.05)。而于免后7个月时分别为69.22172mIU/ml(n=105)及43.76266mIU/ml(n=105),前者极显著的高于后者(t=2.717489,P<0.01)。同时组PPD皮肤阳性率和BCG单独组比较,结果相近,分别为83.66%及75.68%,P>0.05,无显著性差异。利用病例-对照进一步研究表明两组相比免后1个月OR值(95%可信限)分别为4?

【Abstract】 The interaction of simultaneous administration of hepatitis B vaccine(HeVac)and BCG in the newborn had been explored.In comparision of simultaneous groupwith HeVac alone group,the levels of anti-HBs produced at the first month afterimmunization were 27.21049(n=103)and 22.01286 mIU/ml(n=105)respectively,(t=1.117734,P>0.05).At the seventh month,the levels have achieved 69.22172(n=105)and 43.76266(n=105 )mIU/ml,the former being significantly higher than the later(t=2.717489,P<0.01 ).The PPD skin positive rates were similar in simultaneous group andBCG alone group.They were 83.66%and 75.68%respectively(P<0.05).Further study using method of case-control study in epidemiology showed that ORvalues(95%confidence limits)were 4.31(2.34~8.00)and 0.98(0.53~1.79)respectivelyat first month after immunization. The former was significantly higher than the later.x ̄2w=12.986>x ̄210.001=10.828.The OR values at seventh month were 31.36(11.49~104.66)and 4.00(1.82~8. 96),the former being significantly higher than the later.x ̄2w=10.936>x ̄210.001=10.828.The result showed that the simultaneous administration with BCG couldenhance anti-HBs producing ability of the subjects received hepatitis B vaccine. Incomparision of the simultaneous group with the BCG alone group,HeVac could reduce theBCG humoral immunity. The above phenomina is with practical and theoretical significance.
