高血压病患者血糖、胰岛素、血脂及糖负荷后血压的变化

【摘要】 在长沙地区居民中对35名男性高血压患者和35例男性正常血压者进行空腹血糖、胰岛素、甘油三酯、总胆固醇测定及糖耐量试验,观察糖负荷后血压变化。高血压组空腹血糖、胰岛素、甘油三脂、总胆固醇都显著高于正常血压组。空腹血糖和胰岛素水平与血压升高程度关系密切,高血压组中糖耐量异常19/35例,糖负荷后血糖,胰岛素浓度显著高于正常血压组,且60分钟高峰胰岛素时的收缩压与平均动脉压均显著升高。结果说明高血压患者中存在高胰岛素血症、糖耐量异常,血糖和胰岛素均是血压的影响因素。

【Abstract】 We prospectively investigated the serum insulin,glucose and lipoprotein and measured the blood pressure at one hour after oral 75 gram glucose load in 35 essential hypertensive subjects as compared with age and body habitus matched normotensive subjects. In com-paresion with 35 normotensives,the essential hypertension patients: the fasting glucose concentration and insulin level were 6. 62 ±1.82 versus 5. 24 ± 0. 52mmol/L (P<0. 001) and 18.71±5. 72 VS 11. 56±3. 83μ/ml(P<0. 005) respectively, serum triglyceride 1.74± 0. 31VS 1.14±0. 38mmol/L(P<0. 005) ,total cholesterol 5. 72±0. 42VS 3. 76±0. 65mmol/ L(P<0. 001). At one hour after 75g glucose load,the SBP and MBP were much higher than the fasting SBP and MBP(P<0. 001 to 0. 01)respectively,which also correlated with one-hour peak insulin level(r = 0. 427,P<0. 01 and r = 0. 335,P<0. 05). there are 19 case with glucose intolerance in 35 hypertensives. While post-glucose-load the insulin level were higher than normotensive subjects. These findings demonstrate that essential hypertension patients have an hyperinsulinemia, dyslipidemia and glucose intolerance. Blood glucose and insulin were correlated with blood pressure and also were the determing factors on hypertension.
