100例急性白血病患者多药耐药性检测分析
DETECTION AND ANALYSIS OF MULTIDRUG RESISTANCE IN 100 CASES OF ACUTE LEUKEMIA
【摘要】 作者应用抗P-糖蛋白(P-170)单抗JSB-1对100例急性白血病(AL)患者进行了检测,结果表明:(1)包括M3在内的几乎所有AL亚型均可有MDR阳性表达,初诊组MDR表达率以M5(62.5%)为最高,ANLL(20%)虽高于ALL(8.3%),但差异无显著性意义。(2)初诊组部分患者(18.2%)呈MDR阳性表达,提示MDR的内在性。(3)P-170阳性率与AL临床状况密切相关,难治组P-170阳性率(20.6±15.6%)和MDR表达率(78.9%)均显著高于初诊组(7.6±9.3%和18.2%),MDR阳性表达者对化疗不敏感,完全缓解率低,临床预后差。故进行该项检测对指导临床化疗方案设计、预测化疗疗效、判断预后及逆转MDR均有重要意义。
【Abstract】 Abstract Bone marrow specimens from 100 cases of acute leukemia (AL)diagnosed by MIC were detected with fluorescence microscopy for their mdr─1 expression using monoclonal antibody JSB ─1 against p─glycoprotein(P─170).The results showed that almost all subtypes of AL had P─170 expression and M5 of ANLL had a significantly higher expression rate in the newly diagnosed grou.The MDR expression highly correlated with the clinical drug resistance andm prognosi The Positive rate of P─170(20.8% ± 14.9%)and MDR expression(78.9%)ofrefractory group were significantly higher than newly diagnosed group(7.5%±9.8%and 18.2% respectively ).Cases with MDR expression had poor response to chemotherapy and bad prognosis.
