高血压脑卒中病人颈动脉及前臂血管的结构与功能改变

【摘要】 40例高血压并脑血管意外(HT—S)病人用超声检测颈动脉及肱动脉结构与血流动力学,并有Ⅰ期高血压(29例),Ⅱ期高血压(38例),正常人(37例)等3组作对照。研究证明HT—S病人颈动脉粥样硬化斑块发生率为62.5%,无1例管腔狭窄>50%。颈、肱动脉内径明显较其他3组大;颈动脉僵硬度明显较正常人与Ⅰ期高血压病人高,与Ⅱ期高血压病人无区别。Doppler检测HT—S组颈动脉收缩期血流速度峰值(PS),舒张期血流速度(ED),时间—平均血流速度(TAV)最低,阻力指数(RI)最高;前臂缺血后血管扩张能力最低。逐步判别分析表明:年龄、ED与前臂血管最大阻力/最小阻力比是三项决定HT—S的主要因素。结论:超声检查大动脉有可能从血管结构与血流动力学方面为高血压脑血管意外提供预报资料。P<0.01;与I期组比较△△P<0.01;与II期组比较▲▲p<0.01血管内径:高血压病人颈、肱、桡动脉内径均较正常人明显增大。颈动脉内径II期高血压较I期大,而HT—S组最大。由于正常人颈动脉内径随年龄增长而增大。HT—S组平均年龄又较其他3组为高。SBP对颈动脉内径也有影响。经ANOCA检验,矫正年龄影响(P=0.00179)及?

【Abstract】 Abstract Ultrasonic duplex scanning was carried out on 40cases of hypertensive cerebral vascular accident (HT-S) to investigate the changes in structure and hemodynamics of carotid and brachial artery. A cohort of 3 groups of phase I hypertensives (n = 29),phase. Ⅱ hypertensives (n=38) and normotensives (n=37) served as controls. Studies showed that the detecting rate of atherosclerotic plaque in carotid artery in HT-S group was 62.5%, while in no case of them the stenosis greater than 50%. The internal diameters of carotid and brachial artery in HT-S group were greater than that in other 3 groups. The stiffness of carotid artery was significantly greater in HT-S than that in phase I hypertensives and normotensives, with no difference as compared to that of phase Ⅱhypertensives. Doppler ultrasonic detection showed that systolic peak velocity (PS), end diastolic velocity(ED), time average velocity (TAV) and forearm vascular dilatation capacity were the lowest in HT-S group while the resistance index the highest. Stepwise discrimination analysis demonstrated that: Age, ED and forearm vascular dilatation capacity were the tmee most important determinants of hypertensive cerebral accidents. Conclusion: Ultrasonic duplexscanning may provide data for predicting hypertensivecerebral vascular accident.
