应用乙肝血源疫苗和乙肝免疫球蛋白阻断母婴传播的研究
EFFECT OF HEPATITIS B VIRUS(HBV) VACCINE FROM BLOOD AND HBV IMMUNOGLOBULIN (IG)ON PREVENTION OF HBV TRANSMISSION FROM MOTHER TO CHILD
【摘要】 对134例无症状乙肝携带者母亲所生的新生儿,分A组(单纯疫苗组);B组(乙肝疫苗和乙肝免疫球蛋白间隔免疫组);C组(乙肝疫苗和乙肝免疫球蛋白同时免疫组)。根据母血又分单阳性和双阳性,进行实验观察,于免疫后12个月时检测抗-HBs,计算阳转率,双阳性A、B、C组分别为46.00%、96.42%、93.75%。单阳性分别为62.30%、92.30%、94.44%。阻断保护率双阳性分别为41.67%、95.53%、84.37%。单阳性分别为37.50%、71.15%、86.13%。经统计学处理,A组与B、C组有显著差异。而B、C组无明显差异,阻断效果相似,B、C组的明显优于A组。因此,应大力提倡乙肝疫苗和乙肝免疫球蛋白联合免疫措施。间隔或同时联合免疫均可采用。
【Abstract】 The 134 newborns, whose mothers were diagnosed as asymptomatic HBV carriers, were divided into 3(A. B and C) groups. The A group was immunized with HBV vaccine, the B group with HBV vaccine and Ig at different time, and the C group with HBV vaccine and Ig simultaneously. On the presence of HB8Ag and/of HBeAg in their mothers, blood, their newborns of the 3 groups were further divided into 2 subgroups: single positive subgroup (HBsAg+), and double postive subgroup (Both HBsAg+ and HBeAg+). At the 12th month after immunolization, the anti-HBs was detected and the anti-HBs positive rates were cal culated. The results showed that the anti-HBs positive rates of A. B. C group were 46. 00%, 96. 42%, 93. 75% respectively in double positive subgroup; and 62. 30% 93. 3%, and 94. 44% in single positive subgroup, respectively. The prevension efficiency was 41. 67%, 95. 53%, 84. 37% respectively in doubIe positive subgroup; and 37. 50%, 71. 15% and 86.31% in single positive subgroup respectively. Statistical data show that there are significant differences between A and B group ; and between A and C group. Moreover, the efficiencies of B and C groups were much better than that of A group. This suggested that thd HBV vaccine and Ig should be combinatively applied at the same or different time for prevention the transmission of HBV from mother to Child
