【摘要】 为探索心电向量图对慢性肺心病右室肥大的诊断标准,对50例肺心病、32例肺气肿和143例,正常成人的QRS向量进行了系统性研究。结果表明,取Rx<0.5mV、Sx/Rx>0.58、横面S/R>1.1、最大空间向量仰角≥65°为右室肥大诊断标准,对肺心病诊断敏感性为90％,对肺气肿及正常组诊断特异性分别为87.5％和96.5％。更多还原

【Abstract】 More than 30 parameters of the QRS loops of 50 patients with chronic cor put monale (CCP), 32 patients with emphysema, and 143 normal subjects were analysed with the help of the computerized autoanalysis system for vectorcardiography. Four simplified and quantitative vectorcardiographic criteria for the diagnosis of right ventricular hypertrophy (RVH) were derived from 15 useful parameters of QRS loops. RVH was considered to be present when one or more of the following criteria were fulfilled; (1) The magnitude of R wave in the X axis was below 0. 5 mV; (2) The ratio of magnitude of S wave to R wave in the X axis exceeded 0. 58: (3) The ratio of magnitude of S vetor to R vector in horizontal plane was over 1.1; (4) The elevation of maximal spatial QRS vector was equal to or greater than 65 degrees. Application of those criteria achieved 90% sensitivity in patients with chronic cor pulmonale. The diagnostic specificity in patients with emphysema and normal subjects were 87. 3% and 96. 5% respectively. When the criteria recommended by the Third National CCP Symposium was adopted, the sensitivity in patients with CCP reached 86%; the diagnostic specificity in patients with emphysema and normal subjects only reached 65. 63% and 65. 72% respectively.更多还原
