【摘要】 235例腰椎间盘突出症应用腰椎牵引及强的松龙硬膜外注射治疗,少数病例配合造影观察,证实药液进入硬膜外腔隙沿着神经根鞘向下延伸显影。腰椎牵引治疗中,反应较重及牵引结束后疗效不显著的患者作强的松龙硬膜外注射,疗效优于腰椎牵引对照组,P<0.01。腰椎牵引反应重,用硬膜外强的松龙治疗,缩短了治疗时间;并优于牵引结束后疗效不显著的强的松龙治疗组,P<0.01。单侧椎间盘突出其疗效优于中央性椎间盘突出合并椎管狭窄及黄韧带肥厚者。更多还原

【Abstract】 This paper reports an observation on the relative merits of pelvic traction and epidural prednisolone treatment for patients suffering slipped lumbar disk.235 patients were included in this series. Clinical diagnosis was confirmed by X-ray and CT. They were divided into 3 groups: Group 1-tractions, plus epidural injection of prednisolone during one course of traction; Group 2-Epidural prednisolone injection was given after completion of one course of traction: Group 3 - Pelvic traction only. Results could be enumerated as follows:(1) Injected prednisolone could go up from sacral hiatus to the level of lumbar 3-4 along the epidural space. Three injection were recommanded in one course of the treatment. (2) Addition of prednisolone injection was better then traction alone (P<0.01). The curative effect is the best in group 1 (P< 0.01). (3) The curative effect is better for one-side slipped lumbar disk than that of central slipped disk complicated with spinal stenosis and hypertrophy of the surrounding ligaments.更多还原
