单肺通气时改变呼吸频率对潮气量和每分钟通气量的影响

【摘要】 观察17例肺叶切除术的病人,探讨双肺通气(TLV)、单肺等容通气(OLV-等容)和单肺等压通气(OLV-等压)时不同呼吸频率(f)对潮气量(Vγ)、每分钟通气量(VE)及血气值的影响。结果提示,改变f对气道压力(Paw)无影响。f与Vγ呈负相关r=-0.99。而f与VE呈正相关r=0.99。OLV时Vγ较TLV时明显减少,VE也相应减少。若提高f,可使OLV-等容时VE的不足得以缓解。而OLV-等压则不然。OLV时的血气值有所改变,PaO2较TLV降低50%,而PaCO2在OLV等压组升高,通过调整VA可予以纠正。

【Abstract】 eventeen adult patients undergoing selected pulmonectomy were divided into 3 groups according to differentventilation modes:two-lung ventilation(TLV ),one-lung ventilation ( OLV)with equal tidal volume and OLVwith equal airway pressure (Paw).The purpose of study was to observe the influence of changing respiratory fre-quency (f)on tidal volume (Vγ),minute volume(VE)and blood-gas analyse during TLV and OLV. The results:the change of f didn’t affect on Paw. f was negatively related to Vγ(r=0. 99),but positively to VE(r=0. 99).Vγduring OLV was obviously lower than that during TLV, so did VE. If f was increased during OLV-equal volume,the insufficient part of VE could be compensated by increasing f,but it could not during OLV-equal Paw. Blood-gasanalyses showed that PaO2 during OLV was 50%lower than that during TLV and PaCOz during OLV-equal Pawwas higher than that during TLV(P<0.01 )。
