乙肝酵母基因疫苗免疫后抗-HBs下降规律及推测加强免疫时间的探讨
A Study on Kinetics of Anti-HBS Decrease and Prediction of Revaccination Time after Yeast Derived HB Vaccination
【摘要】 200名HBV易感者分二组分别接种Amgen公司生产的乙肝酵母基因疫苗(G苗)和Smith公司疫苗(s苗).4年后24.59%的免疫者抗-HBs滴度下降到10mlU/ml以下。抗-HBs保护水平持续时间取决于3针免疫后最高滴度水平。抗-HBs>1000mlU/ml者,4年后G苗仍100%大于10mlU/ml,S苗则为97.6%,而抗-HBs在10~100mIU/ml之间者,4年后皆<10mIU/nl。抗-HBs滴度随时间延长呈指数曲线下降,首次免疫后前24个月几乎下降了全部滴度的80%,以后下降缓慢,抗-HBs滴度水平不同组其回归直线斜率相似,表明抗-HBs滴度下降百分率与最初浓度无关。在此基础上制出标准回归直线,并用做平行线的方法推测个体加强免疫时间。
【Abstract】 200 Young People were vaccinated with two kinds of Yeast -derived HB vaccines (G, S) and followed up for 4 Years, in order to explore the persistence and the kinetics of anti-HBS and predict the optimal time for revaccination. The result showed that 4 Years after the first vaccination, anti- BHS titres in 24.59% of vaccinees had fallen below 10mIU/ ml. The persistence of anti-BHS above this value depended on its maximal antbody titres after the third vaccination. In vac- cinees with maximal anti-HBStitres above 1000mIU/ml all re- cipients vac-cinated with vaccine G and 97.6% of recipients with vaccine S still had titres above 10mIU/ml after 4 Years. whereas no one with initial titres between 10 and 100mIU/ml maintained anti-HBS Soncentrations above 10mIU/ml for longer than 4 years. The anti-HBS titres decreased in the form of exponential curve. The fall in anti-HBS tires was rapid at first, leading to 80% reduction in the first 24 months after the first vaccination. Subsequently, the decrease was slower. The slopes of linear regression in anti-HBS deferent groups were similar by changing their curves into straight lines. This indi- cated that the percentage of anti-HBS desresase was indepen- den of the initial concen tration of anti-HBS. On the basi sof this, a theoretical linear regression was described and predict- ed the opimal time for an individual reveaccination by drawing a paralled line with theoretical line.
