间歇主动脉根部顺行灌注合并用冠状静脉窦堵塞——血液动力学及心肌超微结构观察
INTERMITTENT ANTEGRADE AORTIC ROOT CARDIOPLEGIA COMBINED WITH CORONARY SINUS OCCLUSION: OBSERVATION OF HEMODYNAMICS AND ULTRASTRUCTURES
【摘要】 本实验旨在研究间歇性主动脉根部顺行灌注联合应用冠状静脉窦堵塞法在冠状动脉梗阻时对1小时缺血心肌的保护作用。结果表明,此法可使冷停搏液逆行灌注入梗阻动脉辖区心肌;对该区心肌降温低(15℃);心肌超微结构保护良好;缺血后心功能恢复较优,心脏指数和左室搏出功指数分别恢复至缺血前的128~142%和115~158%。与间歇性主动脉根部顺行灌注联合应用冠状静脉窦堵塞法相比,单纯主动脉根部顺行灌注保护缺血心肌效果较差。
【Abstract】 The study was aimed to evaluate the protection of 1-hour ischemic myocardium in the presence of coronary artery obstruction by intermittent antegrade aortic root cardioplegia combined with coronary sinus occlusion (APCSO). The results showed that APCSO could perfuse retrogradely into the myocardial region supplied by the occluded coronary arteries, and provide hypothermia (15℃) with a better protection of the myocardial ultrastructure of that region, Postischemic cardiac function of APCSO recovered much better, in which cardiac index and left ventricular stroke work index were 128~142% and 115~158% of the preischemic value, respectively. Compared with APCSO, antegrade aortic root cardioplegia alone provided less protection for the ischemie myocardium.
