阿米洛利对充血性心力衰竭患者血清地高辛浓度的影响

【摘要】 本文观察10例服用地高辛14日(0.25mg/日)的心力衰竭患者,血清地高辛浓度(SDC)达稳定状态后联用阿米洛利5mg、10mg各14日。期末SDC分别增高29.61%(P<0.01)和26.75%(P<0.01)。SDC增高,似与阿米洛利的剂量大小无关,而与疗程有关,但联用2周后SDC可达稳定状态。故两药联用时应密切注意观察临床表现和SDC变化,防止出现不良反应。本文还对SDC升高的机制作初步的探讨。

【Abstract】 Ten inpatients (mean age of 52 years) suffering from congestive heart failure were treated with digoxin and amiloride for 42 days. According to the NYHA classification of cardiac function, 5 cases were of class Ⅳ, 4 cases of class Ⅲ, 1 case of class Ⅱ. The observation was divided into three stages with two weeks in each. All of the patients were given digoxin 0.25 mg q. d. in the first stage, and amiloride was added in doses of 5mg and 10mg q. d. in the second and third stage, respectively. Serum digoxin concentration (SDC), electrolytes, BUN, Cr, Clcr and ECG were studied. SDC was increased by 29.61% at the end of the second stage (p<0.01 vs. first stage), by 29.13% and 26.75% on the 7th and 14th day of the third stage, respectively (p<0.01) vs. first stage). No significant difference was found in the increase of SDC between the second and third stage. It shows that the increase of SDC is not related to the dosage of amiloride. Serum potassium and BUN concentrations were increased but Clcr decreased、after amiloride treatment.
