无损伤性指套式光电容积振动法测定血压的可靠性
A STUDY ON NON-INVASIVE MONITORING OF BLOOD PRESSURE IN HUMAN FINGER BY THE VOLUME OSCILLOMETRIC METHOD AND ITS RELIABILITY
【摘要】 本文对15例冠状动脉造影的心脏病人,应用光电容积振动法,无损伤性监测手指动脉压力并与同时从升主动脉测得的内压力进行比较。表明两种方法在收缩压、平均动脉压、舒张压之间的相关系数r分别为0.8,0.7,0.6(p<0.001)。手指法测定收缩压和平均动脉压与升主动脉内压力分别相差-0.06与~1.0kpa,舒张压相差0.16kpa,收缩压和舒张压在两种方法间无显著性差异。3名肾上腺嗜铬细胞癌病人,在手术过程中手指法测得的指动脉压力和同侧桡动脉压力,也呈现高度相关性。研究表明,UV-101手指血压监护装置重复性能良好,误差率低于2%,因此,手指法测定的血压能可靠反映动脉内压力,其操作简便,实用,是长时间连续无损伤性监护血压的有效方法。
【Abstract】 A new non-invasive blood pressure monitoring device(UV-101) based on volume oscillometric technique using transmittance infrared photo-electric finger plethysmograqhy(TIPP) was evaluated in 15 patients who underwent a coronary arteriography.Linear regression was used to compare(he blood pressure values from finger arterial pressure with simultaneous aorta arterial pressure:systolic,mean arterial pressure and diastolic pressure measured by the common catheterization method,the correlation coefficient for 45 pairs of measurements from all subjects were r = 0.8,0.7 and 0.6 respectively(n = 45,p<0.001).Finger measurements values were lower than the direct aorta systolic and mean pressures(mean difference:-0.16 Kpa and-1.0 Kpa respectively),whereas the diastolic pressure difference was 0.16 Kpa by an.average.The systolic and diastolic pressure values by two methods were not statistically different.It was demonstrated that the relative and absolute values of blood pressure in fingers can be extrapolated to the intra-arterial pressure by an appropriate equation.The coefficient of variation CV of the measurements with the finger device(UV-101)was less than 2%,showrng thus a good reliability.During the operaton,indirect finger pressure measurement using the BP monitor devive(UV-101)was compared with direct arteria radialis pressure measurement in 3 patients with Pheochromocytoma,the correlation coefficient were excellent between the two methods.The non-invasive volume oscillometric(TIPP)instrument is an easy and useful device for continuous blood pressure monitoring in situations where brachial measurement would be inconvenient.
