胃癌及癌前期病变PNA、ConA受体和CEA表达研究
Immunohistochemical Study of CEA and Receptors of PNA, ConA in the Gastric Cacinoma and Pre—Cancerous Lesions
【摘要】 应用癌胚抗原、花生凝集素、刀豆凝集素对12例单纯慢性萎缩胃炎、12例腺上皮中度异型增生病变、23例重度异型增生伴癌变、30例浸润进展型胃癌组织,做了检测,观察其阳性染色强度及分布模式异常。结果表明,3种标记物的阳性强度、分布模式异常均随着细胞异型增生程度的增高而增高,并有显著差异。发展至浸润进展型癌,CEA的强阳性率达80.0%,胞浆内分布达72.4%,ConA的强阳性率为83.3%,胞浆内分布85.7%,PNA的强阳性率为53.3%,胞浆内分布达64.3%。并发现癌巢旁间质中亦有PNA阳性反应,提示与肿瘤浸润扩散有关,为此这3种标志物可用于决定癌变的一种常规辅助病理诊断手段,提高对早期癌变的诊断率。
【Abstract】 Using three biological markers(CEA, PNA, ConA), four groups of the gastric lesions were studied immunohistochemically, including simple atrophic gastritis(12cases), chronic gastritis complicated by moderate dysplasia(12cases), severe dysplasia by cancerous changes (23 cases), and advanced gastric carcinoma(30 cases). By the assays of the staining extension and pattern, the results showed that the rate of the staining abnormality was in accordance with the degree of epithelial dysplasia, When the lession has developed to the advanced stage of carcinoma, the strong positivity rates of CEA, ConA, PNA were up to 80%, 83.3%, 53.3% respectively. The rates of positive intra—cytoplasmic staining pattern also were high. Besides, the PNA positive reaction was discovered in the stroma near the nests of carcinoma. This phenomena revealed there exists relationship between PNA positive substance and tumor’s infiltration. Through this study, we suggest that throe markers of CEA, PNA, ConA can be used in the routine pathologic diagnosis to detect the early malignant changes of the gastric epithelium.
