氨茶硷静脉滴注治疗3°房室传导阻滞

【摘要】 本文12例冠心病伴3°房室传导阻滞(3°A-VB).其中5例有阿斯综合征(ASS)发作。以氯茶硷静脉滴注治疗,总有效率75%。对有诱因、急起发作的效果更好,避免了心脏临时起搏。疗效机制:根据腺苷对心脏的作用是通过心肌细胞表面受体介导产生负性变时、变导和变力作用.而氨硷与腺苷对心肌细胞 A1受体起竞争性拮抗.逆转了腺苷介导的3°A-VB.转为正常房室传导。氨茶硷可作为希司束上阻滞的首选治疗。

【Abstract】 This paper reported 12 cases of clinical coronary heart disease with third de-gree atrioventricular block(3°A-VB),and among them,5 cases were complicated by Adam-Stokes Syndrome.They were therapied by aminophylline(500 mg in 5% glucose solution 250ml)with intravenous drip.The rate of cure was 75.0%.This method of therapy in 3°A-VB wasespecially effective in the patients who had inducements or immediate attack,and thus cardiacpacemaker was saved.The action of adenosine appeared to mediate the negative chronotropic,dromotropic(e.g,A-V block)and inotropic actions through the interaction of receptors presentin cardiomyocytes.Aminohylline is an adenosine antagonist,it competitively antagonazes car-diomyocytic A1 receptor-mediated actions of adenosine(e.g,A-V block),therefore it can recov-er normal atrioventricular conduction.Aminophylline may be used as the first-choiced drugs forthe treatment of 3°A-VB.
