肺炎与肺炎心衰儿童的血浆肾素、血管紧张素Ⅱ、醛固酮浓度测定及其临床意义

【摘要】 本文采用放免法测定76名婴幼儿肺炎血浆肾素、血管紧张素Ⅱ、醛固酮浓度并与正常小儿对照,自身治疗前后进行比较证实,肺炎组三项指标明显高于对照组;肺炎心力衰竭小儿三项指标高于肺炎组。各型肺炎治疗后均有不同程度的下降。肺炎与肺炎心衰肾素—血管紧张素—醛固酮系统活性增高可能是形成外周阻抗和钠水潴留的重要因素。抗 RAA 系统的药物治疗可能是有效的。

【Abstract】 Seventy—six cases,at infancy or childhood,with pneumonia were included in this study.Of these patients the levels of plasma renin activity,angiotensin Ⅱ and aldosterone were measured with radio—immunoassay,and monitered during the medication against pnemonia.The results showed that the serum levels of these three parameters were elevated significantly in the pneumo- nia group compared with the control,and reached to the highest in the children with pneumonia related heart failure.As the children recovered from the disease through medication the serum lev- els of these parameters declined concomitantly. It was demonstrated that in sick children with pneumonia the activity and sensitivity of the renin—angiotensin Ⅱ—aldosterone(RAA)system may be the important factor in inducing phe- ripheral vascular resistance and sodium and water retentim,enhanced blood vessel resistance were induced sequentially.It is suggested by this study that in dealing with children pneumonia,the ad- ministration of RAA system antagonists seems to he rational.
