负荷诱发定量检测室壁运动异常早期诊断冠心病的研究

【摘要】 本研究对11例正常人及21例早期冠心病患者,进行大剂量潘生丁——超声心动图(大剂量 DET)定量检测室壁运动,并与核素心肌显像的运动靶心图定量对比。结果发现21例患者平静超声心动图,8例(38.1%)检出区域性室壁运动异常(RWMA),潘生丁负荷后20例(95.24%)检出 R WMA。21例患者共420个节段中,大剂量 DET 定量和运动靶心图定量节段检出率高度一致,表明大剂量 DET 定量检测室壁运动异常为早期诊断冠心病的方法。

【Abstract】 To assess usefullness of quantitative dipyridamole echocardiography test (DET) in detecting coronary artery disease (CAD) early,by means of computerized image processing technique,the quantitative analysis of left ventricular regional wall motion abnormalities (RWMA) induced by a large dose of the dipyridamole echocardigraphy test has been performed in the detection of early CAD (absence of clinically documen- ted CAD).Twenty patients who had nontypical angina pectoris and nonspecific ST segment abnormalition,but show reversible myocardial ischemia regions determined by exercise Tc99m-MIBI SPECT,regional wall motion were compared with the quantitave analysis of exercise Bull′s eye plot in myocardium perfused SPECT.400 regions wrer evalu- ated with both technique,of 111 regions with myocardium perfused defect,90 (83%) regions had RWMA;of 125 regions with RWMA,90 (95.24%) had myocardium perfused defect.Result between both showed good correlation in practice,to detecting CAD,can apply alternatively or both.
