实验性高胰岛素血症大鼠的动脉病变观察

【摘要】 为探讨绝对增高的胰岛素(insulin,INS)水平对动脉壁的影响,本实验给正常大鼠每日皮下注射INS(20u/kg体重)12周。光镜下全部主动脉均见内膜灶状增厚,病灶主要由平滑肌细胞(smooth muscle cell,SMC)组成,并且灶内有脂滴;透射电镜显示内膜和中膜浅层SMC增生,内皮下间隙加宽,纤维、基质增多及内弹力板的破坏;扫描电镜见列单个内皮细胞(endothelial cell,EC)隆起或数个EC呈小丘状隆起,“火山口”样缺损和偶见EC脱失。实验表明绝对升高的INS水平能引起主动脉早期动脉粥样硬化(atherosclerosis,AS)病变。本文探讨了高INS血症促进早期AS形成的机理。

【Abstract】 To discuss the effectiveness of absolutely elevated insulin level on atherosclerosis, insulin (20μ/kg weight, day) was given to normal rats for 12 weeks. All fo-cusses in the aortic intima were thickened and the focus was formed by proliferation of smooth muscle cells, and lipid droplets were observed under the microscope. Transmission electromicroscopic observation showed smooth muscle cell proliferation in both intima and the inner layer of media, subendothelial space widening, fiber and matrix increase and dis-truction of the internal elastic membrane. Scanning electromicroscopy showed elevation of single endothelia cell or elevation of several endothelial cells in the shape of hills, moutain-mouth-like defect, and disappearance of endothelial cells. The results suggest that absolutely elevated insulin level can cause early atherosclerosis. The mechanism that high insulin level promoted the formation of early atherosclerosis was discussed.
