Edwards套棒法复位胸腰椎爆裂骨折的力学分析与实验研究
AN EXPERIMENTAL STUDY WITH BIOMECHANICAL ANALYSIS ON THE CORRECTION OF THORACOLUMBAR BURSTING FRACTURE WITH EDWARDS ROD-SLEEVE INSTRUMENTATION
【摘要】 本文分析了Edwards套棒装置三点加压复位胸腰椎爆裂骨折的力学原理,并通过建立力学模型导出脊柱所受横向前推力量与套棒厚度成正比,前纵韧带拉伸复位力量与横向前推力量成正比。使用Edwards套棒不仅提高后伸复位力矩和轴向矫正力量,而且也有助于克服残留后凸畸形角,满足脊柱不同节段的生理弧度要求,使爆裂椎体获得动力性复位。生物力学实验结果显示单侧Edwards套棒方法产生89.0N轴向复位力,较直HarringtOn棒提高82.7%。
【Abstract】 The correcting mechanism of thoracolumbar bursting fracture by Edwards rod-sleeve instrumentation was described as three-point compression, Mechanical relations between the transverce force and the thickness of rod-sleeve, and between the axial correction force and transverse force were proved to be indirect proportion by establishing force models. The use of Edwards rod-sleeve instrumentation can increase correcting hyperextension moments and axial correction forces, and also decrease postoperation kyphosis. Biomechanical study showed that Edwards rod-sleeve instrumentation could apply 89.0 N axial correction force while used unilaterally on flesh swine thoracic and lumbar spines which Were 82.7% higher than 48.7 N by Harrington distraction rod.
