遗传与营养代谢参数和少年期血压

Genetic Factor and Nutritional/Metabolic Parameters in Relation to the Blood Pressure in Adolescents

【摘要】 择四个16~19岁两性少年群,对无高血压家族史(149人),父或母患高血压(137人)、父母均患或父(母)并有祖或外祖父(母)患高血压(118人)的三组,测血压、身长、体重及血浆载脂蛋白A1及B、总蛋白、总胆固醇、糖化血红蛋白、尿酸、血糖和24h尿Na/肌酐(Cr)、K/Cr、Ca/Cr、Mg/Cr等。发现:各血、尿营养代谢参数在各少年群的水平不一;有高血压家族史组尿Ca高、尿Mg低:遗传因子明显影响血压,且其遗传中也有体重指数遗传的介入;与在成年期所见相反:血总蛋白与血压、尿内尿素氮均呈负相关,身长、体重与尿Na、Ca、Mg等亦呈负相关。上述营养代谢参数,在少年期除营养摄入外,还可能受发育代谢率的影响,故不能完全反映营养素的摄入水平。

【Abstract】 Four groups of both sexes, 16-19 years were enrolled comparing the blood pressure (BP), height, body weight and plasma apo-protein A1 and B, total protein, cholesterol, glycogenated hemoglobin, uric acid, glucose and 24h urinary Na/creatinine (Cr), K/Cr, Ca/Cr, Mg/Cr, etc. in 3 seperate divisions (225 male, 179 female): those without hypertensive genetic predisposition (149 subjects); with either father or mother hypertensive (137 subjects), both parents or one parent and one patternal or maternal grand parent hypertensive (118 subjects). Results showed: blood and urinary variables were quite diverse in different groups; the urinary Ca was higher while Mg lower in the group with genetic predisposition; the genetic factor was an important determing factor of BP, and the inheritance of body mass index (Q. I.) was also implicated, blood total protein was associated inversely with BP and urinary urea nitrogen, the end metabolite of protein, and height, body weight and Q. I. correlated inversely with urinary Na, Ca, Mg, being wholly contrary to those in adults. Therefore, there are also developmental and metabolic factors exerting effects upon the parameters listed above beside the dietary intake. Thus these nutritional parameters in adolescents can not be taken as an reflection of nutritional intake accurately.
