多普勒超声心动图预测慢性肺病患者肺动脉高压

【摘要】 对20例慢性肺病患者,在做右心导管测肺动脉压的前后48h内,应用多普勒超声心动图检测肺动脉收缩期血流频谱,探讨频谱形态和各流速时间间期及其指数与肺动脉压的关系。结果发现肺动脉高压患者肺动脉收缩期血流频谱顶点提前,血流加速时间缩短,血流加速时间与肺动脉平均压呈明显负相关(r=-0.892,P<0.001),提示多普勒超声心动图是预测慢性肺病患者有无肺动脉高压及其程度的简单而可靠的方法。

【Abstract】 In this study, Doppler echocardiography and right heart catheterization were performed in 48 hours on 20 patients with chronic pulmonary disease. The Doppler data were analysed including the right ventricular pre-ejection period (RVPEP), the pulmonary flow acceleration time (AT), the right ventricular ejection time (RVET), and the relationship with pulmonary artery pressure. We found patients with pulmonary hypertension (PAH) showed a rapid acceleration and early deceleration with a sharp peak in early systole, their RVPEP was significantly longer, and AT and RVET were significantly shorter than those of the patients without PAH. The sensitivity for AT<100 ms predicted that PAH was 92.86%, and the specificity 100%. The result also showed a good line correlation between AT and pulmonary artery pressure (γ=-0.982, P<0.001).
