尼群地平对猪急性低氧肺动脉高压的降压作用和对血流动力学的影响

【摘要】 本文报道小剂量尼群地平静脉注射观察其对急性低氧性肺动脉高压(猪)的降压作用及其对血流动力学的影响,结果显示:小剂量尼群地平静注能降低急性低氧所致肺动脉高压,降低肺循环血管阻力,其降压作用超过6小时,并能减轻右心负荷,增加氧的输送能力,对心肌和体循环动力学无明显影响,进一步深入研究尼群地平的作用,有希望提供临床作为降低肺动脉高压的药物使用.

【Abstract】 As pulmonary vasoconstriction is a majior factor producing pulmonary hypertension duing hy- poxia.It can be Inhibited or attenuated by calcium channel blocking agents,we evaluated the ef- fects of nitrendipint on the hemodynamics of pulmmonary and systemic circulation in seven pigs. Animals were anesthized with intravenous injection of pentothal(10 mg/kg),and vetilated with a Servo-900 C ventilator.The hemodynamics of pulmonary or systemic circulaion were continuously monitored by IVY 410 monitor via a Swan-Ganz and arterial catheter.Minute ventalaion and inspi- rated oxygen concentration were adjusted to maintain a stable PaO2 and PaCO2,cardiac output(CO) systemic pressure and pulmonary arterial prepare were measured during normoxia (PaO214.95± 0.78 kpa)hypoxia (PaO25.97±0.35 kpa) and hypoxla with nitrendipine injection (100 ug/kg iv).During 15--16% oxygen Ventilation the mean pulmonary anry pressure (PAPm),pul- monary vasocular resistnce (PVR),right ventricle work index (RVWI) increased all P<0.05. After a single dose of 100 ug/kg of nitrendipne iv,PAP dropped from 4.2±0.19 to 3.18±0.17 kpa,PVR from 46.3±7.1 to 32.2±3.7kpa,L/s RVWI from 17.8±2.4 to 15.4±2.5 J/m2 all P<0.05.Co(L/min) rose from 5.09±0.66 to 5.6±0.72.This response lasted more than six hours.Oxygen delivery also improved.Meanwhile systemic pressure remained unchanged.The data suggests that nitrendipine release,hypoxia induced pulmonary hypertension and improves oxy- gen transport in pigs.
