14C-尿素呼吸试验检测胃幽门弯曲菌
BREATH TEST FOR THE DETECTION OF CAMPYLOBACTER PYLORI INFECTION IN THE STOMACH
【摘要】 本文建立改良14C-尿素呼吸试验。试验以5μCi14C-尿素与50mg非标记尿素加入高浓度奶粉溶液中同服。呼气分别在0、7、15、30、45、60、90分钟直接收集于1mmol氢氧化海胺中。呼出的14C量直接以dpm/mmolCO2表示。实验显示,在试餐后7至90分钟内,确诊为幽门弯曲菌阴性18例其dpm值均在200以下,而阳性17例则均在200以上;两者比较,在试餐后测定之任何时相,差异均有高度显著性(P<0.05)。14C呼出高峰期在试餐后30分钟,因此可以第30分钟呼气的dpm值大于200作为判断14-尿素呼吸试验之阳性标准。本文进而对46例呼吸试验结果与胃窦及胃体活检标本的培养及组织学检查结果进行比较,计算出呼吸试验的敏感性为100%、特异性为95.5%,阳性预测值为96.0%,阴性预测值为100%。 该试验具有简易、可行、准确的优点,适于作为胃幽门弯曲菌感染流行病学调查之用。
【Abstract】 We have developed a modified 14C-Urea breath test in which the subjects were given 5μCi 14C-urea (135KBq) with 50 mg non-radioactive urea added in high concentration powder milk solution. Breath samples were collected in 1 mmol hyamine hydroxide at 0. 7. 15. 30, 45. 60 and 90 min. The amount of 14C at these times were expressed directly as dpm/ mmol CO2. The results revealed that all the values of dpm were below 200 in all the 18 C. pylori negative patients, but all above 200 in all theI7C. pylori positive patients at any time between 7 and 90 min. The difference between negative and positive patients was highly significant at all time points after test meal (P<0.005). The peak time of 14C exhaled was at 30 min after testmeal, therefore a peak value of 200 dpm at 30 min after testmeal was established as the cut off value between a positive and negative result. Our study also compared the outcome of the breath test to the results of culture and histology of endoscopically obtained antrum and body biopsies in 46 patients. The breath test discriminated the 24 positive patients and the 22 negative patients for C. pylori very well. The calculated sensitivity was 100%. specificity 95,5%, positive predictive value 96.0%.and negative predictive value 100. We conclude that this modified 14C-urea breath test is a simple noninvasive test that has excellent sensitivity and specificity for detection of Campylobacter pylori infection in the stomach and thus is well suited for epidemiological studies in the general population.
