大骨节病区学龄儿童尿硒和肌酐指数及其关系初探
Relationship Between Urinary Selenium Excretion and Crcatininc Indces of Children from Kaschin-BeckDisease Affected and Unaffected Areas
【摘要】 本文测定了40例病区儿童(男性7~13岁)和19例病区服硒儿童的24小时尿硒排泄量(μg/kg/24hr)和肌酐指数(mg/kg/24hr),以42例非病区儿童作为对照,结果表明,病区儿童尿硒和肌酐指数均明显低于、服硒病区儿童接近于远离病区的非病区儿童,而且各组的两指标间呈显著性正相关(r=0.81,P<0.05);受试儿童以尿硒重新分组,不管是病区组内,非病区及服硒组内,还是全部受试者,肌酐指数均随尿硒水平的升高而增加,呈显著性正相关;尿硒和肌酐指数轻度降低率(<16mg/kg/24hr)和重度降低率(<13mg/kg/24hr)的相关系数分别为+0.89和+0.86,以此计算两率的ED50尿硒量分别为0.20和0.10μg/kg/24hr,大约分别是大骨节病患病率ED50尿硒量的2倍和4倍。这些说明低硒首先导致肌肉代谢的改变和病区儿童肌酐指数的降低,补硒可有效地改善病区儿童的肌肉病变。
【Abstract】 Se excretion in 24 hours’ urinary and crcatininc indeces of 40 children from Kaschin-Bcck disease affected areas and 19 Se-supplemented children in an endemic area were determined with 42 children from non-endemic areas as controls. Results showed that the two indexes for endemic children were lower than those of non-endemic ones, and the indeces of the Se-supplemented were similar to those of non-endemic ones.Analyses of urinary Se excretion of endemic, non-endemic children or the whole subjects suggested that creatinine indeces gradually increased with the rise of urinary Se and that there were significantly positive correlations between the two indeces at all occasions. The correlation coefficient between urinary Se and the slightly decreased rate, and seriously decreased rate of creatininine indeces were +0.89 and +0.86, respectively. Urinary Se ED50 of the rates were 0.20 and 0.10μg/kg/24hr,respectively, calculated by above data, which were about 1 and 3 times more than the urinary Se ED50 of Kaschin-Beck disease prevalence rate.These indicated that Se-deficience initially led to the change of muscular metabolism and then caused decrease of creatinine indeces in patients with Kaschin-Beck disease.
