【摘要】 1982年4月至1986年11月,我们共诊治750例颈椎病,有150倒在颈椎侧位X线照片中发现颈椎前纵韧带骨化（OALL）占20％,男79例,女71例,其表现状态分为单骨化、双骨化、多骨化、单鸟嘴、双鸟嘴、骨挢、跨跃及混合八个类型;在150例中,132例无吞咽困难,18例产生不同程度之吞咽困难,（占本组总颈椎病数为2.4％）其中8例有严重吞咽困难。同时合并气管刺激,及压迫、上胸部闷胀、胸部不适、恶心呕吐、声音嘶哑及间歇性呼吸困难,占12％。我们通过钡剂食管造影、CT、及用GIFP₃型食管内窥镜检查,以资确诊。10例进行非手术综合治疗,余8例固症状明显,经足够的保守治疗无效而施术。本文讨论了:（一） “OALL”型颈椎病的命名,我们通过生理解剖、生物力学,典型X线改变、明显临床症状等说明发病原因,并提出“OALL”型颈椎病的命名依据。（二） “OALL”型颈椎病的诊断及鉴别诊断应与食管癌、骨赘、第二次骨化中心未愈合及颈椎类风湿病作鉴别。（三） “OALL”型颈椎病的治疗,除非手术外,本组手方法有:①单纯骨化块切除术者1例,②“OALL”块切除加病椎椎间盘切除加椎间植骨术者3例,③“OALL”块切除加病椎椎间盘切除加椎后骨刺刮除加椎间植骨术者4例。8例手术皆收到满意效果。更多还原

【Abstract】 From Apil 1982 to Novemher 1986 we treated 750 cates of cervical spondylosis,of which 150 cases had ossification of the anterior longitudjnal ligament（OALL） （20％）。In these 150 cases, 18 patients gave a history of dysphagia symptoms et al, and roentgenographic studies （including bariun meal）, endoscopy （GIFP₃） and CT scan showed esophageal compression. It composed of 12％ "OALL" cases and 2.4％ of the total cases of the cervical spondylosis. Morphological appearance of "OALL" in lateral roentgenographic studics & barium meal showed: 48 cases of simpe "OALL", 32％, 15 cases of double "OALL", 10％; 14 cases of multiple "OALL", 9.3％; 14 cases of simple bird beak, 9.3％; 2 cases of double bird beak, 1.3％; 35 cases of hone birge, 23.3％; 2 cases of alternating changes, 1.3％; 20 cases of mixed type, 13.3％. 18 cases complicated with dysphagia et al. 10 cases of them were troated conservatively and 8 cases came to operation.This article discusses:Nomination of "OALL" of cervical spondylosis, the diagnosis and differential diagnosis methods of the operative treatment simple removal of the "OALL", 1 case removal+discectomy+intervertebral bone graft fusion, 3 caseso removal+discectomy+removal of the posterior osteophytes+intervertebral bone graft fusion, 4 cases.
