【摘要】 本文介绍了移植部分下唇做为翻转皮辦,对丧失上唇特有外形的唇裂病人重建上唇和人中的手术方法。手术的基本原则如下。(1)无论应用何种方法,都必须把移植的唇瓣放置在上唇的中间,而且其形状和范围必须与人中相符。三角形者显得难看,因而不宜应用。(2)移植唇辦的肌层必须恰当地与口轮匝肌缝合,如果上唇肌层的肌肉不整,在移植前必须整复肌肉的排列次序。(3)如遇双侧唇裂,必须全部切除红唇,充分展平移植唇辦,然后才能二次修剪瓣的人中范围。(4)在移植后的二次手术中,必须修整上唇的垂直长度和人中,要塑造弓形的上唇、小突和人中窝。这个手术应间隔6个月后进行。更多还原

【Abstract】 A new operative technique was introduced to reconstruct the upper lip totally with the philtrum in the center by transplanting a portion of the lower lip as a cross lip flap for the cleft lip patient whc-had lost the characteristic shape of the upper lip.Fundamentals of the procedure are as follows:1.The transplanted lip flap must be placed in the center of the upper lip regardless the method to be used,and its shape and size must be suitable to the philtrum.The triangle shape looks ugly and therefore inadequate. 2.The muscle layer of the transplanted lip flap must be sutured correctly with the orbiclaris oris muscle,if there is disarrangement of the muscle layer of the upper lip,the muscle alignment must be corrected before transplantation.3.In the case of bilateral cleft lip,the prolabium must be resected entirely and a wide lip flap should be transplanted first,and then the flap can be trimmed to the size of the philtrum secondarily.4.In the secondary operation after transplantation,the vertical length of the upper lip and irregular line of the philtrum must be adjusted.The cupid’s bow,tubercle,and philtrum dimple must be also formed.The final finish-up operation should be performed after an interval of six months.更多还原
