应用主成分分析评价肺心病急性发作期化瘀的治疗作用
Evaluation of Curative Effect of Stasis-Removing Therapy during Acute Attack of Cor Pulmonale by Means of Principal Component Analysis
【摘要】 通过对109例慢性肺心病急性发作期瘀证患者应用中医古方治疗前后8项血液流变学指标测值的主成分分析,认为小青龙汤具有降低ηp、Fib,改善 EPM 的作用麻杏石甘汤具有降低ηb、ηb-1/Ht、ESR、K 的作用;千金苇茎汤具有降低 ESR、K、Ht,提高EPM 的作用;真武汤具有降低ηb、ηb-1/Ht、Ht 的作用,从而达到良好的化瘀效果。结果提示,上述血液流变学指标组合的改变,分别与中医寒饮、肺热、痰热、寨水等病机有一定的内在联系。
【Abstract】 The effect of treatment for blood stasis syndrome of chronic cor pulmonale during acute attack with oral administration of Xiaoqinglong decoction (XD,小青龙汤),Maxingshigan decoction(MD,麻 杏石甘汤),Qianjinweijing decoction(QD,千金苇茎汤)and Zhenwu decoction(ZD,真武汤)was studied in 109 cases through principal component analysis of the 8 hemorheologic indexesbefore and after the treatment.The results showed that the curative effect of the 4 decoctions respectively were as follows:(1)XD was effective in reducing the plasma viscosity,blood coagulation and improving the electrologic feature of erythrocyte membrane;(2)MD performed an action to decrease the blood viscosity and erythrocyte agglutination;(3)QD aided to reduce the erythrocyte agglutination and blood condensation,and to improve the electrologic features of erythrocyte membrane;and(4)ZD proved helpful in diminishing the viscosity and condensation of the blood.The above-mentioned 4 decoctions are not conventional recipe for activating the blood circulation and removing the stasis. However,through ascertaining the pathogenic factors by syndrome differentiation and thereafter deciding proper treatment to overcome these pathogenic factors,they all shówed satisfactory effect in removing the stasis when used to treat"cold fluid-retention","lung-heit","phlegm-heat"and "cold-edema"respectively.The improvement of physical and chemical features of the blood by these decoctions is one of the microcosmic mechanisms of their stasis-eliminating effect. Analysis on the data of the hemorheologic indexes suggests that there exist certain inner links between the changes of index combination of plasma specific viscosity(~ηp),fibrinogen level(Fib), RBC electrophoretic mobility(EPM)and"cold fluid-retention";between the changes of combination of blood specific viscosity(~ηb),blood reduction-viscosity(~ηb-1/H_t),hematocrit(H_t)and"cold-edema"; between the changes of combination of ~η,~ηb~81/H_t,erythrocyte sedimentation rate(ESR),ESR equational coefficient K and"lung-heat";and between the changes of combination of ESR,K,H_t, EPM and"phlegm-heat".
