【Abstract】 During the application of Clomiphene Citrate (CC) to induce ovulation, it is very important to use B-ultrasound monitoring. Through the observation of the number and size of the follicles, one can decide the time of administration and adjust the dose of human chorionic gonadotropin (HCG) , and prevent the occurrence of ovarian hyperstimulation syndrome and polypregnancy.Twenty-one CC-indured cycles in 10 infertile women with abnormal ovulation cycle and 18 spontaneous cycles in 12 fertile women with normal menstrual cycle were examined by B-ultrasonography. Normal and abnormal ultrasound images in induced ovulatiodn cycles were observed and the patterns of follicular growth between induced and sponta neous cycles were compared. The results are as follows.1. Fifteen normal induced cycles:The average growth velocity of the follicles(2.3±0.4mm per day, X±SD) and the average diameter of the maximum follicles before rupture(21.8±2.0mm) of normal indued cycles were not significantly different(p>0.05) from those of the spontaneous cycles, which were 2.2±2.0mm per day and 21.3±2.1mm, respectively, whereas the incidence of multiple follicles is significantly higher than that of the latter.2. The six abnormal induced cycles are of 3 categories;(1) In 2 cycles, the follicles are poor and grow slowly. These follicles ruptured when the diameters were less than 17mm.(2) In 1 cycle, luteinized follicular cyst was formed.(3) In 3 cycles, ovarian hyperstimulation syndrome was observed. In these cycles, more than 3 follicles grow rapidly and their diameters ware greater than 25mm and the maximum diameter of ovary may reach 70mm.The authors believe that (1) In induced ovulation cycles, if ultrasonography showed images of poor follicular growth, CC dose should be adjusted in the next cycle. (2) The best time for HCG administration is the time when the follicular diameter is 20±2mm, (3) For preventing the occurrence of ovarian hyperstimulation syndrome and polypregn<-ancy, administering HCG must be stopped when the follicular diameter is greater than 25mm or when 3 follicles or more with diameters of 20-25mm were seen.更多还原
