中国胆道外科早期的成都经验
FIRST EPISODE OF CHINESE BILIARY SURGERY——CHENGDU EXPERIENCE IN 1940s
【摘要】 抗日战争时期,华西、齐鲁、中央三大学联合医院外科于1942年,在成都发现并成功地手术治疗了第一例胆道蛔虫病,改变了不能在急性期施行胆囊手术的传统观点。自此到1946年间,建立了该病的诊断和治疗常规,并先后发现肝内胆管结石、胆管狭窄和脓性胆管炎等病在中国的特点,门静脉内胆栓也被发现,这些经验为发展中国胆道外科作出了最早的贡献。
【Abstract】 The first historical successful operativeremoval of ascaris worm from the bile ductwas performed in Chengdu, China, byDrs. Yang Jia-Liang and Yang Zhen-Huaon December 15, 1942 under the advocacyof Dr. Dong Bingqi (PC Tung), Head ofthe Surgical Department of the UnitedHospital of West China, Cheloo and Cen-tral Universities during the period of Anti-Japanese War. Before this, billary opera-tions were performed very scarcely as onlyindicated for elective patients. Almost allpatients were admitted in acute stage andthey usually refused operation when theacute pain was over. Recommendation ofemergency operation was against the sur-gical rule at that time, but was later onproved to be remedial for biliary infec-tions. Since then, up to 1946, a routine pro-gram of diagnosis and treatment of biliaryascariasis was established. Subsequently,intrahepatic gallstones, strictures of bileducts, acute obstructive suppurative chol-angitis, etc. were found to be character-istic of Chinese cholangiopathy. Bile em-boli in portal vein was also firstly observ-ed in Chengdu. These early experiencesmade good contributions to the develop-ment of Chinese biliary surgery in lateryears.
