未控制糖尿病患者下丘脑-垂体-甲状腺轴调节功能的损伤

【摘要】 本文观察了77名糖尿病患者和46名正常人血清甲状腺激素与TSH对TRH兴奋试验的反应。在未控糖尿病中重现了低T3和高rTa血症,且TSH对TRH的反应减低,空腹血糖与T3(r=-0.31,P<0.05)、BTSH(基础TSH,r=-0.52,P<0.01),TSH(TSH增值、r=-0.42,P<0.05)及θTSH(曲线下面积,r=-0.39、P<0.05)呈负相关。长期控制的糖尿病患者之TRH兴奋试验均正常,提示血糖越高,下丘脑-垂体-甲状腺轴的损伤越明显。TRH兴奋试验可作为反应糖尿病长期控制状态的指标。

【Abstract】 Serum thyroid hormones, TSH and TSH response to TRH stimulation test in / / diabetics (DM) and 46 normal subjects were studied.Impaired TSH response to TRH stimulation test was found in the uncontrolled DM besides low serum T3 and high serum rT3. Moreover, an inverse correlation between the fasting plasma glucose and T3 (r = -0.31, P<0.05), BTSH (basal TSH, r= -0.52, P<0.01),△TSH (expressed as TSH increment, r = -0.42, P<0.05), and θTSH (areas under the curve, r = -0.39, P<0.05) in the uncontrolled DM. It suggested that the degree of hypothalamo-hypophysial-thyroid axial impairment be parallel to that of hyperglycemia.When the fasting plasma glucose was well controlled below 130 mg/dl, the TSH response to TRH stimulation test might be normalized. Therefore, TRH stimulation test may be used as an index in demonstrating the controlled state of DM.
