【摘要】 本文总结31例胰岛细胞肿瘤的治疗经验。过细的病史、必要的化验配合、术前术中的肿瘤定位及全面细致的术中手法探查是手术成功的关键。无功能性胰岛细胞瘤早期诊断较难,B超与CT是诊断的主要手段。它多数位于胰头,是低度恶性肿瘤,应扩大手术范围及长期随访。术中未能找到肿瘤者可作术中胰静脉血IRI测定或门静脉分段采血测定IRI及术中应用B超定位。除非证实为弥漫性胰岛增生者,尽量少作盲目性胰切除术。更多还原

【Abstract】 Twenty-seven functioning and 4 nonfunctioning islet cell tumors were treated surgically between Jan 1973 and June 1986. Among the 27 functioning tumors, 25 were benign insulinoma, 1 malignant insulinoma and 1 hyperplasia of islet cells. All of them were easily recognized clinically by their characteristic ’Whipple’s triad’. The diagnosis of nonfunctioning ones depended much on ultrasound and CT examination.The preoperative localization of the tumors within pancreatic tissue is emphasized. Unless generalized hyperplasia of islet cell is proved, blind resection of the tail part of pancreas is not recommended.更多还原
