用改良游泳法收集精子作人工授精治疗男性不育症
COLLECTION OF MOTILE SPERMATOZOA WITH A MODIFIED SPERM SWIM-UP TECHNIQUE FOR AIH IN THE MANAGEMENT OF MALE INFERTILITY
【摘要】 本文介绍了一种改良的烧杯-高渗F10游泳法,用以收集活动精子,经等渗F10洗涤后行宫腔内授精以治疗男性不育症。9名不育男子(5例弱精、1例少精、3例原因不明不育症)接受治疗。为了提高活动精子回收率,采用烧杯作为精子游泳的容器,以扩大精液与F10溶液间之界面及缩短精子进入溶液的游程。高渗F10溶液渗透压为380mosm/1它可能利于除去精子表面的获能抑制因子。精液处理后,即得到精子密度高、活力强、正常形态率高的悬液。5例弱精症患者的活动精子百分率从29±13提高到85±12(SD)%。活动精子回收率为43.7±10.7%。在B型超声波监测下,于排卵前一天宫腔内人工授精一次。2例弱精症和1例原因不明不育症患者妻子怀孕,其中1名弱精症患者的妻子已于1986年6月生一健康女婴。作者认为,对于某些不育症,用这种改良游泳法收集精子是值得推荐的。
【Abstract】 This paper presents a modified sperm swim-up technique, i.e., the beaker-hypertonic F 10 method for the collection of motile spermatozoa in 9 infertile men (5 asthenospermic,1 oligospermic and 3 without apparent causes), and then the sperms were inseminated to their spouses intrauterinely after washed with isotonic F 10.In order to increase the retrieval rate of motile spermatozoa, a beaker was used as the container for sperm swim-up to increase the area of interface between the semen and the F 10 medium and to shorten the course of sperm swim-up. The hypertonic F 10 medium with a osmotic pressure of 380 mosm/l may contribute to the elimination of the decapacitation factors attached to the sperm’s surface. After such treatment, a suspension containing an adequate amount of motile spermatozoa with a high rate of normal morphology was obtained, the average motility of 5 asthenospermic men being raised from 29±13 to 85±12(SD) % with a motile spermatozoa retrieval rate of 43.4± 10.7%.Intrauterine insemination was performed one day before ovulation as monitored by B-ultrasonography.The spouses of two asthenospermic men and one whose husband has no apparent cause of infertility got pregnant. One of the asthenospermic men’s wife has born a healthy female infant in June 1986. The authors believe that in the management of certain kinds of male infertility,the present modified method seems to be recommendable.
