肝硬变病人的垂体肾上腺轴机能

【摘要】 <正> 血浆皮质醇水平的变化反映肾上腺皮质的分泌和组织器官对皮质醇的降解转化作用。生理状态下下丘脑-垂体-肾上腺轴控制皮质醇的合成和分泌。血浆皮质醇浓度升高抑制垂体促皮质素(ACTH)的释放。起激素自身负反馈控制作用。生理状态下下丘脑-垂体-肾上腺轴调节控制作用使昼夜间的血浆皮质醇水平呈节律性变化。肝脏是糖类皮质素作用的主要靶器官和降解转化场所。我们曾报告了各种肝病时血浆皮质醇的变化,本文又进一步观察了肝硬变病人血浆皮质醇昼

【Abstract】 For study the function of pituitary adrenal axis,we observed the day and night rhythmic secretion of adrenocorticosterol and the responses of plasma corticosterol to ACTH and dexamethason administration in patients with decompensated hepatic cirrhosis (N=21).The rhythmic secretion of corticosterol was present, but the level of plasma corticosterol at 8: 00 Am.was lower in the cirrhosis group than that in the control group (8.64±4.21 vs 14.92+4.96ug% p<0.01).The response peak of plasma corticosterol appearted to be eight hours after ACTH administration in control group, whereas it was not present until after 16 hours in the cirrhosis group. The response peak values of ACTH was also reduced in cirrhosis group (21.02±8.41 vs 35.84±9.74ug% p<0.05).Four of ten patients were escaped the inhibitary effect of dexamethason on corticcosterol secret ion.The data suggested that functional disorder of pituitary adenal axis be present in patients with cirrhosis.The results were considered to be decreased function of adrenocortex and the defect of hormonal degradation in cirrhosis.
