经皮穿刺气囊导管二尖瓣成形术(7例报告)

【Abstract】 Seven Patients with rheumatic mitral stenosis (RMS) were treated by PBMV from Nov. 1985 to Jan. 1986. Among them, 4 were male and 3 were female. The age ranged from 15 to 51 (mean 34.1) years, All the patients their valves were thickened, which were mild or marked motion limited without calcification.The diameter of the orifice of mitral valve was 8.3-2.1mm with cardiac capacity grade II-III or IV. The balloon diameter was 2-3mm less than that of the mitral annulus was checked by 2-dimensional echocardiogram(2DC).The balloon was placed at the mitral orifice by. using percutaneous technique via femonal vein and transsepta of ventricles of. heart to left ventricle. The balloon was inflated to a pressure of 1.5-2kg/c.m~2 for 3 seconds. If the balloon, dilatation catheter (BDC) can not insert into the left ventricle (LV). A special guider was placed in the descending aorta through atrial septum, left atrium (LA), LV and ascending aorta by using a flow-directed catheter. Then the BDC was inserted through the special guider:The patients toelelrated the procedure well without major complications. PBMV was sucessful in all patients. The symptoms disappeared immediately following PBMV. The diastolic thrill disappeared in 4 patients. The mean LA pressure measured was 22.7±2 and 10.0±4.4mmHg before and after PAMV(P<0.01). The gradients across the mitral valve decreased from 17.7±7.8 to 5.6±5.9mmHg (P<0.01). 2DC showed the mitral are increasing from 1.4±0.4 to 2.0±0.4cm~2 (P<0.01).We conclude that the preliminary results of PBMV demonstrated is a hopeful treatment with safe, minimal injury and satisfactory results for patients with RMS.
