S—100蛋白在软组织肿瘤病理诊断中的价值

【摘要】 40例软组织良恶性肿瘤用ABC免疫组化法显示S—100蛋白,均采用福尔马林固定,石蜡包埋的组织。结果:良性雪旺氏细胞源性肿瘤(雪旺氏瘤、节细胞神经瘤、颗粒细胞瘤)均显S—100蛋白强阳性。多数恶性雪旺氏瘤(8/10)亦显阳性,但一般反应较上述良性肿瘤要弱。由黑色素细胞衍生的恶性黑色素瘤和软组织软骨肉瘤S—100蛋白均显强阳性,相反地,由组织细胞及纤维母细胞衍生的纤维组织细胞瘤及纤维肉瘤S—100蛋白均为阴性。 通过观察认为S—100蛋白对下列肿瘤的鉴别确诊有帮助:(a)鉴别雪旺氏瘤与纤维组织细胞瘤。 (b)鉴别恶性雪旺氏瘤与纤维肉瘤。(c)鉴别恶性黑色素瘤与癌,梭形细胞肉瘤,特别是少或无黑色素的恶性黑色素瘤。 S—100蛋白在某些恶性雪旺氏瘤中缺如或很少,被认为与组成肿瘤的细胞为“反分化”细胞有关,反分化细胞丧失合成S—100蛋白的能力。

【Abstract】 40 benign and malignant soft tissue tumors were studied for thepresence of S-100 protein by means of Avidin-Biotin-peroxidase Complex (ABC) technique on formalin-fixed paraffin-embeded tissue. Virtually, all benign nerve sheath tumors(neur-ilemoma, ganglioneuroma, granular cell tumor) showed strong staining of S-100 protein. Most of the malignant sehwannomas (8 / 10) were stained positive also, but the degree of staining was weaker than that of the above-mentioned benign tumors. S-100 protein was identified within melanocytic tumors (malignant melanoma) and chondrosarcoma of soft tissue (with strong staining), on the contrary, the tumors derived from histocyte and fibroblast such as dermatofibrosarcoma protuberans, fibrosarcoma were negative for S-100 protein.For the diagnosis of sofe tissue tumors, S-100 staining was proven to be helpful in the differential diagnosis of: (a) benign schwannoma from fibrous histocytoma (b) ma lignant schwannoma from conventional fibrosarcoma. (c) malignant melanomas, especially those with little or no melanin pigment ,from carcinoma or spindle cell sarcoma.S-100 protein was rare or absent in some malignant schwannoma. It was suggested that most of the cell components of these tumors were "dedifferentiated cells " , that have returned to their embryonic state lost the capability of synthesis of S-100 protein.
