关于天津市300例小儿缺铁实验室诊断标准的探讨

【Abstract】 Laboratory studies concerning iron status of 300 children lived in Tianjin wasinvestigated in 8 nurseries,kindergartons and some scattered children in Tianjin urbandistricts from 1984 to 1985.Six items of laboratory values were obtained from the 300cases who have received iron therapy.i.e.Hb,Hct,SI,FEP,SF,and FEP/Hb.1.The data showed significant difference among different age groups(P>0.05),but not in different sex groups.Based on these findings we got 6 reference normalvalues of the above items for children of 6 month to 3 year age group.2.Patients with only abnormal FEP were dynamically observed before and afterreceiving iron therapy.Among them,29 patients(9.7%)had normal SF.After they hadreceived iron therapy,their FEP almost reached the normal level.These explain that,if the diagnosis of iron deficiency depended only on change of SF as much as 9~10%of the patients would be misdiagnosed.So we suggest that children with only anabnormal FEP level should be diagnosed as iron deficiency.3.For simplicity,in clinical usage,we use only a common normal value for the6 month to 3 year old age group in the diagnosis of ID.The normal value is in accordancewith that of other investigators.The maximum level of the normal FEP is 38.9 μg/dl,which is significiently lowerthan that of other regions reported in China.However,it approaches to 35 μg/dlreported by Rayrip ect(Rayrip’s values are based on 4,160 children cases in the generalsurvey).
