高温中暑时血流动力学的改变及其机制的探讨——Ⅱ.高温中暑时循环衰竭机制的探讨
INVESTIGATION ON THE MECHANISM OF CIRCULATORY FAILURE DURING HEAT STROKE IN DOGS
【摘要】 在20只麻醉的杂种狗身上对高温中暑时产生循环衰竭的机制进行了探讨。对照组当直肠温度升至41℃以上时,心搏量与心输出量明显降低(P<0.05)。但在40.5℃静脉注射纳洛酮的动物,41℃时的心搏量与心输出量却明显增加(P<0.05),同时中心静脉压较对照组下降得更多,平均动脉压在注射后稍有升高。此外,在对照组的6只动物上于体温38℃与42.5℃时,分别测定了血清钠与钾的浓度。血清钠增加36.3±12.32mg%(P<0.05),血清钾增加6.3±1.61mg%(P<0.01)。结果提示;高温中暑时的循环衰竭可能与β—内啡肽有关;在体温42℃以上时出现的心律不齐,可能与血清钾升高有关。
【Abstract】 The mechanism of circulatory failure during heat stroke was investigated in 20 mongrel dogs under pentobarbital aneasthesia. When the rectal temperature rised to 41℃,the stroke volume and cardiac output decreased significantly ( P<0.01, n = 10). Naloxone < 0.1-0.3mg/kg ) was injected intravenously at 40.5℃ in the experimental group. After injection both stroke volume and cardiac output increased markedly at 41℃ ( P<0.05, n = 10) . They showed increase still at 42℃, but there was no statistical significance. The central venous pressure in thes eanimals with naloxone treatment decreased more markedly than that in the control group. The results suggest that P-endophin may take part in the development of circulatory failure during heat stroke. In addition, the serum concentrations of sodium and potassium were measured in six dogs of the control group. Potassium cocentration increased by 6.3±1.61mg% (P<0.01 ) .The arrhythmia during hyperthermia beyond 42 degree centigrade may be related to the increased potassium concentration
