【Abstract】 Thirty seven cases of trilogy of Fallot were treated surgically from June 1972to June 1984 in our hospital.They are 23 females and 14 males aged between 3to 27 years after the operation respectively.Two patients died 3 hours and 48hours.Hospital mortality rate was 5.4%.One died of severe acidosis and theother of sudden death in I.C.U.Operation were performed with hypothermia on 8 cases,29 cases with the aid oftotal cardiopulmonary bypass.Pulmonary valvular stenosis of 0.1 to 0.5 cm~2 wasfound in 9 cases,0.6 to 1.0 cm~2 in 15 cases,and 1.1 to 1.5 cm~2 in 13 cases.There were domed pulmonary valve in 31 cases and dysplastic pulmonary valvein 6 cases.The atrial septal defect of ostium secundum or patent foramen ovalewas 1.5 to 4.0 cm in diameter.Thirty cases of the artrial septal defect of ostiumsecundum was repaired by simple suture with silk and the other 7 cases were notrepaired.Anaesthesis should be shortened starting from the step of pulmonary valvectomyin severe trilogy of Fallot complicated with 3～4 class heart dysfunction.The operation was carried out through median sternotomy.Cardiopulmonary by-pass is instituted only through the vena cava superior and the vena cava inferiorwhich are blocked up.Direct vision of the valvular stenosis through the main pulmonary artery in pre-fered for the valvulotomy.The heart will keep working due to the sustained blood supply from the coronaryartery if the cardioplegia is not applied.更多还原
