蕈样肉芽肿(MF)细胞的特征及其诊断意义
CHARACTERISTICS OF THE MYCOSIS FUNGOIDES CELLS AND ITS DIAGNOSTIC SIGNIFICANCE
【摘要】 本文结合印片、电镜、免疫酶标等检查和临床随访,重点观察85例MF不同皮损和淋巴结中MF细胞的形态;明确了MF细胞系来源于转化过程中外周T辅助细胞,并指出其谱系形态变化。MF细胞并非仅见于MF,但为诊断MF的必备条件。
【Abstract】 This paper reports the observations on the morphology of MF cells in 112 specimens of skin lesions in various developmental stages and 3 specimens of lymph nodes from 85 cases of MF. In 14 cases examinations of electron microscope and tissue imprints using cytochemical staining were simultaneously studied. The tests of rosette formation with sheep crythrocytes (Es) and mouse erythrocytes (Em) and determination of cells with immunoglobulins on the cell surface membrane (SmIgG, SmIgM) for cell suspensions in 4 cases were performed. 7 specimens from 6 cases were also studied by immunoperoxidase method (ABC) using monoclonal antibodies (Leul, Leu2a, Leu3a, Leu4, la, B1, OKM1, T3, Te, κ, λ)It seems now certain that MF cells are derived from peripheral helper T cells during the process of transformation and their changes of morphology and size are in form of a spectrum ranging from small uniform cells to clear twisted nuclear cells, clear round nuclear cells, T-immunoblasts and bizarre giant cells. There is a correlation between the pathologic subtypes, clinical stages and morphology of skin lesions in MF. Small uniform cell type and small polymorphic cell type are mainly seen in premycotic patches. Comjon or large polymorphic cell type is mainly seen in infiltrating plaques. Large polymorphic cell type with some small or medium clear round cells or bizarre giant cells are mainly seen in highly infiltrated plaques or tumours.
