肱骨干骨折外固定的受力分析
Mechanical Analysis for the Local External Immobilization of Fracture of Humeral Shaft
【摘要】 中西医结合治疗骨折在临床上已取得良好效果,并已积累了丰富的经验。但对夹板固定中的受力分析研究的较少。本文对18个正常人体上臂在夹板的约束下,在肘关节被动屈曲90°和主动屈曲时,用环式传感器测定了布带的拉力,用板式传感器测定了纸压垫下的压力以及夹板对肢体的压力。给出了夹板对上臂的压力分布曲线,弯矩变化曲线,夹板的挠曲线以及相对最大挠度。结果表明:夹板下的压力主要集中在纸压垫下,垫压力随垫厚及垫位而改变,肱二头肌的收缩使夹板下的总压力和垫压力分别增加82%和76%。
【Abstract】 since 1958 excellent clinical results and abundent experience have been achieved in the treatment of fractures by the integrated metho d of Chinese traditional medicine and western medicine. However, few articles on mechanical analysis of local splint immobilization have been reported. we measured the tensile force of clothe swathe with ring-like transducer and the reaction compressive force of arm and paper pads acting on the splint with plate transducer on 18 normal human arms immobilized by splints with its elbow joint at passive flexion and during active flexion (biceps contraction) at this position. Curves of distribution of compressive force on inner surface of splints, deflexion curve of splint as well as its relative maximum deflexion, and diagrams of bending moment of splint have been obtained. The resultsshowed that the compressive force under splints mainly concentrated to paper pads, and varied with the thickness and position of pads as well as contraction of biceps brachii. The total compressive force and padé compressive force under splints increase up to 82% and 76% respectively during biceps contraction.
