尿液鲎试验在尿路感染中的临床意义
CLINICAL VALUE OF LIMULUS TEST OF URINE IN THE DIAGNOSIS OF URINARY TRACT INFECTION
【摘要】 鲎试验于1970年用于检测人的内毒素血症。自1980年开始,我们采用定性法和定量法尿液鲎试验来诊断尿路感染。共检查了829例次尿液标本。本文比较了尿路感染组和非感染组尿液鲎试验的结果,两组差别有极显著意义,并对尿液鲎试验和细菌学的关系,以及在异体肾移植方面鉴别排异和感染中的应用作了讨论。初步体会尿液鲎试验对革兰阴性细菌所致的尿路感染的诊断具有灵敏度高,不受使用抗生素影响等优点。
【Abstract】 Limulua test (LT) was introduced in 1970 for the detection of endotpxin in the blood of patients with sepsis due to gram-negative baoillus. Since 1980, we have used qualitative and quantitative LT of urine in the diagnosis of urinary tract infection. A total of 829 urine samples, including 418 from patients with urinary infection and 411 from patients without such infection, were examined. The positive rate of LT of urine in these 2 groups of patients were compared. The difference was highly significant (P<0.001). The bacteriological aspect of LT of urine and its application in differentiation rejection from infection in patients after renal homotransplantation were also discussed. Our preliminary experience with LT of urine indicated that it was a sensitive method for the diagnosis of gram-negative bacterial infection of urinary tract, and unlike urine culture, the result was uneffeoted by antibiotic treatment even if the pathogenic organism had been inhibited or killed in a short period.
