小儿肾脏病血清循环免疫复合物及补体测定的临床意义

【摘要】 本文报道了71例急性肾小球肾炎(AGN)血清循环免疫复合物(CIC)及134例肾脏病血清补体(C3)和免疫球蛋白(Ig)的测定结果。在AGN的初期,血清CIC显著高于正常儿童,C3显著低于正常儿童。随疾病的恢复,GIC渐降,C3渐升,两者呈完全性负相关。AGN中CIC显著升高伴C3显著降低者,病情严重、病程长;血清CIC持续不阴转伴有C3持续低值者预后不佳。肾病时除IgM偏高外,IgG、IgA均降低。本文提示:(1)AGN的发病机理主要是由IC所致;(2)单纯性肾病与细胞免疫有关;(3)测定血清CIC和C3均有助于肾脏疾病的鉴别诊断、观察病情及判断预后。

【Abstract】 This article reported the results of the determination of circulating immune complex (CIC) in 71 cases of acute glomerulonephritis (AGN) and those of serum complement (C3) and immunoglobulins(Ig) in 134 cases of nephrosis. In the initial stage of AGN the level of CIC was remarkably higher than that of normal children, but C3 was lower. That is to say, the level of CIC was almost always associated with negative C3. When CIC was remarkably higher with a notably low level of C3, the case of AGN was serious,and its course might be comparatively longer when CIC was persistent with a low level of C3, showing that the prognosis was poor. In nephrosis IgM was slightly higher, but IgG and IgA were all low. The article suggested that: (1) AGN is caused by CIC; (2) Nephrosis is related with cell immunity; (3) The determinations of CIC and C3 are useful for differential diagnosis, observation of the disease course and prediction of the prognosis of nephrosis.
