成人急性白血病骨髓细胞的生长动力学探讨
Growth Pattern of Bone Marrow Cells in Adult Acute Leukemia
【摘要】 用姐妹染色单体交换术(SCE)及3H-TdRLI%法测定了骨髓相正常的非白血病患者骨髓15例(对照组)及不同病期的成人急性白血病患者骨髓21例(实验组)。结果证明:经72小时培养后,未经治疗的急性白血病患者骨髓细胞仅有3%的中期分裂细胞完成两个分裂周期。而对照组则有40%的中期分裂细胞可完成2~3个分裂周期。用SCE术与3H-TdRLI%法测定的结果一致,均证明急性白血病细胞的增殖活力明显低于对照组。未治组骨髓细胞为PHA(+)者。即在培养体系中加PHA后可使骨髓细胞总的分裂细胞数(M)或M2+M3%增加。此类病人用以S期特异性药物(包括羟基脲和阿糖胞苷)为主的方案疗效较好。骨髓细胞为PHA(-)者,即培养体系中加入PHA后,骨髓细胞M或M2+M3%减少。则此类患者用周期非特异性药物(三尖杉酯碱和L-门冬酰胺酶)疗效为佳,以上结果提示白血病细胞生物学特性对治疗效应有重要影响。
【Abstract】 Sister chromatid exchange (SCE) analysis & 3H-TdR LI% determination are used for detecting the growth pattern of bone marrow cells of 15 cases of nonleukemic patients (control group) & 21 cases of adult acute leukemia (AL) at different stages (test group). The results show that in pretreated AL only 3% of the metaphases can run 2 cell cycles, while in nonleukemic bone marrow about 40% metaphases can run 2-3 cell cycles within 72-hr incubation. Both SCE analysis & 3H-TdR LI% data reveal that the proliferation of AL cells are at a lower rate. If the pretreated bone marrow shows a PHA(+) reaction, i. e. the total metaphases count(M) or M2 + M3% increases with the addition of PHA in the culture medium, the patient shows good response to the protocol in which S-phase specific agents (HU & Ara-C) are included. In the case of PHA( -) reaction, i. e. either M or M2 + M3% decreases in the presence of PHA, the patients has a better response to phase nonspecific agents (Haccingtonin & L-asparaginase). This implies that the biological features of leukemic cells can play a significant role in their response to antileukemia agents.
