【Abstract】 By using extracorporeal circulation, hypothermia, and cold potassium-magnesium cardioplegia we had performed open heart operaions for 15 cardiac patients, including 9 cases of congenital heart disease and 8 cases of rheumatic heart disease. Specimens of myocardium of ventricles were taken before the application of hypothermia; after occlusion of the aorta; and after rewarming of the patient for pathologic study electron microscope.1. Fibrosis of the intercellular structure of the myocardium in the specimens of rheumatic heart disease is much more frequent than those of congenital heart disease. Changes in specimens taken after aortic occlusion and before rewarming in rheumatic heart disease is more prominent (P<0.05) than that of congenital heart disease which can even be negligible.2. Mitochondria are the most important organelles that are closely related to the recovery of the function of the myocardium.3. Hypothermia and cold potassium-magnesium cardioplegia make the myocardium quickly arrests in diastolic state, by which the reserve of the myocardium is better preserved and therefore in favour of the restoration of the function of the heart.4. Hypothermia around 15-18℃ and aortic occlusion as 80 minutes are prefered for myocardial preservation in open heart surgery for both congenital and rheumatic heart diseases.
