房室传导阻滞的电生理定位及其它定位指标的探讨

【摘要】 <正> 希氏束近端阻滞(BPH)预后良好,一般不需要起搏器治疗,而希氏束内阻滞(IHBB)及希氏求远端阻滞(BDH)预后不良,可有突然晕厥甚至猝死的危险,需要安置永久型人工心脏起搏器。特别是其中的IHBB在心电图(ECG)上QRS波群不增宽,与BPH相同,而预后及治疗与BPH有本质的区别。因此,房室传导阻滞(AVB)的定位诊断,

【Abstract】 The data about the localization of different sites and degrees of atrioventricular block (AVB) in 29 patients, including 11 cases of Intra-His bundle block (IHBB), some results of atropine test and the observation of "septal" q waves in this groupe were offered in this paper. In conclusion, our study shows:1. Atrop nc test has the practical value in the localization of AVB.2. The prediction of IHBB based on the disapperance of "septal " q waves in escape QRS complex alone in patients with AVB and with an escape rhythm of narrow QRS morphology is net reliable.3. Some His bundle electrogram (HBE) findings which have not been described in the previous papers may indicate the extensive and different degree lesions within the His bundle or may indicate the widening of H wave produced by contact with the catheter.4. Under the indication of atropine test the more careful exploration for split H during the procedure of HBE record may increase the diagnosis rate of IHBB.
